Athletics Registration
ATHLETIC CLEARANCE AND PHYSICALS
If you are playing Fall, Winter or Spring 2023-2024 sports seasons you must complete the online athletic clearance process. The process will become much easier as you will only have to enter most information once - annual physicals and any changes to insurance, households or other will have to be updated as needed.
Athletics Registration for the 2023-2024 seasons will be accepted on the FamilyID beginning June 1, 2023 (any physicals dated prior to June 1, 2023 will not be accepted). You must be cleared before you can participate in try-outs, practice or your specific sport season.
Requirements for Athletics Participation
Student-athletes are not allowed to participate in any capacity until this entire process is completed.
PLEASE NOTE: Physicals must be done by a physician (MD) or doctors of osteopathic medicine (DO). Chiropractors, Physician Assistants and Nurses are NOT authorized to sign physical forms.
- Register with FamilyID
- Upload Physical Form: An annual physical form is required of all student-athletes. It MUST be uploaded in FamilyID (do not attach physicals from previous years. Physicals dated prior to June 1, 2023 will not be accepted. We do not accept hard copy forms. Again, PLEASE NOTE: Physicals must be done by a physician (MD) or doctors of osteopathic medicine (DO). Chiropractors, Physician Assistants and Nurses are NOT authorized to sign physical forms.
- Emergency White Card: This card is for Head Coaches to have on hand in the case of an emergency at an away event. Please obtain this card from your Head Coach or the Athletic Office. This card will be turned into your Head Coach.
